Due to the COVID-19 pandemic we kindly ask you to download the COVID-19 checklist below, fill it out and email it back to us 2-3 days prior to your appointment. If you develop any COVID-19 symptoms such as fever, shortness of breath, cough, diarrhea, etc, please call our office at 604-689-5101 and let us know prior to your appointment. If you cannot reach us, stay home and call your healthcare provider.
